These options may provide you with the help you need more quickly.
How to request an appointment
You can request an appointment:
- using an online form
- through the NHS app
- by phoning the surgery
- or in person
If you are unable to use our online form, you can call the surgery. We treat all requests in the same way, no matter how you contact us. Once we've reviewed your request, we'll contact you during surgery hours.

Your care and safety are our first priority. We carefully review all requests and will get back to you during surgery opening hours.
We use the information you give us to decide:
- how quickly we need to see you
- the best person to help you
- whether an in-person appointment is the best option
In this way, we make sure that the people who need our help the most, get it first. Please do tell us if you feel that you need an appointment urgently. You should also tell us if you need any special help during your appointment.
Please also tell us if there's a specific health professional you want to speak to. We'll do our best to fulfil your request. Please bear in mind there might be a longer wait if the person you've asked for is fully booked or away.
You can contact us online using our "Help with a medical issue" form. This form is available on the web, and in the NHS App. It is usually available between 8am and 5pm, Monday to Friday. The form is sometimes switched off for safety reasons.
Please do not use the "Admin Query" form for medical requests.
Types of appointment
We offer the following types of appointments:
Urgent appointments
Please tell us if you feel that you need an appointment urgently. We'll review your request carefully. Where appropriate, we'll offer you an appointment on the same working day, or the next day. If we think you need immediate help, we may tell you to go to a hospital.
When the surgery is closed, please call 111 or go to 111.nhs.uk.
Routine appointments
If you don't need immediate help, please let us know when you request an appointment. This may be, for example, because:
- you're due to have a health check-up or screening
- you need a vaccination
- your ailment is minor, or you've had it for a long time already
Where appropriate, we'll arrange an appointment for you in the next two weeks. If you've asked to see a specific person, there may be a longer wait where they are fully booked or away.
Evening and weekend appointments
We provide evening and weekend appointments outside of our normal opening hours. You can request these by contacting the surgery or by calling 111.
Home visits
If you cannot leave your home, please let us know when you request an appointment.
Help during your appointment
When you request an appointment, please tell us if you need specific help, for example:
- if you cannot come to the surgery
- if you need an interpreter
- or if you have any other access or communication needs
Cancel an appointment
If you cannot come to your appointment, please cancel it as soon as possible so that we can help another patient. You can cancel an appointment:
- by following the link in the appointment confirmation
- using our online form or
- through the NHS app
Urgent advice: Call 111 if you need urgent advice
NHS 111 can help if you have an urgent medical problem and you’re not sure what to do. To get help:
- go to 111.nhs.uk (for people aged 5 and over only)
- call 111
Available 24 hours a day, 7 days a week.
Immediate action required: Call 999 or go to A&E in an emergency
Medical emergencies can include:
- loss of consciousness
- an acute confused state
- fits that are not stopping
- chest pain
- breathing difficulties
- severe bleeding that cannot be stopped
- severe allergic reactions
- severe burns or scalds
